Primary Care Liaison Teams (PCLT) - introduction
Primary Care Liaison - is the name for Bristol's Community Mental Health Teams which are now supported by specialist functional teams (see below) and can now offer greater support to and liaison with the Primary care sector (e.g. GPs). They:
- are multi disciplinary teams offering specialist assessment, treatment and care to adults with mental health problems in their own homes and the community.
- provide effective, evidence-based treatments to reduce and shorten distress and suffering.
- assist service users and carers in accessing such support, both to reduce distress but also to maximise personal development and fulfilment.
- provide advice and information to service users, families and carers.
- establish effective liaison with local Primary Care Team members and other referring agents to shape referrals and support local care.
Each sector's Primary Care Liaison Teams cover separate clusters of GP practices and Health Centres
